Release and Waiver Form I hereby for myself, my heirs, executors, administrators, waive and release any and all rights and claims for damages I may have against the Meeteetse Recreation District, Shoshone National Forest and their agents for any injuries or illness suffered by me which may result from my participation in either of the 5K, 10K, or 15K mountain runs. I attest and verify that I am physically fit to compete in and complete this 5K, 10K, or 15K mountain run event.
